• Notify the NDIS promptly of a participant's death as they cease to be an NDIS participant. Methods of notification include:
  • in-person
  • phone
  • email or letter
  • fill in a form
• Provide the participant's date of death.
• If known, provide the name of the executor of the estate.

This information will be used to finalize the participant's NDIS record. Finalizing the record means ending the plan and discontinuing funded supports.
